Ovarian Serous Carcinoma. Additional Work is Necessary

Recently SGO and OCRA recommended that, even women without a genetic risk, should consider removing the fallopian tubes, and it would be better if the ovaries are also removed, to lower the incidence of ovarian cancer. Since I started working on ovarian cancer I have always believed that most extrauterine serous carcinomas start in the ovary, not in the fallopian tube. This is based on animal studies, daily review of cases, and also when we try to explain the known facts about ovarian cancer. However, the main issue is not where extrauterine serous tumors start, but why they start. In 1998 I was able to induce similar ovarian tumors in guinea pigs with steroid hormones. Since that time, during the daily review of ovarian neoplasms, in many cases, I have found indirect evidence of some correlation between these tumors and steroid hormones. The fallopian tube theory started in 2000 and significant funds have been devoted to the study of the fallopian tube. Salpingectomy, and salpingo-oophorectomy will lower the incidence of these neoplasms. However, Gynecologic Societies and private funds, should also support additional research trying to identify the main reason why these neoplasms start.
