Ovarian Cancer. The significance of Endosalpingiosis

  • To improve the prognosis of ovarian cancer we first need to understand the disease. Low-grade serous tumors in the ovary and implants in the peritoneum start with endosalpingiosis(E), usually a  round lesion with a secretions inside, and originate from a mesenchymal-epithelial transition (fere ex nihilo). This is different from mesothelial inclusions, elongated, flat lesions without secretions. Also, the most common site of endosalpingiosis outside of the ovary is the omentum, which is the most common site of "metastases", supporting the concept of multicentricity. Endosalpingiosis is a hormone induced lesion.
